EmblemHealth Diabetes Prevention Program Delivers Results

EmblemHealth Neighborhood Care’s Community Diabetes Wellness Program sees positive early results and looks to the future

– EmblemHealth, one of the nation’s largest not-for-profit health insurers, launched its first Community Diabetes Wellness Program in the Bronx at its Neighborhood Care Fordham Road center. Fordham Road is one of 15 centers that provide resources and support to some of the most underserved neighborhoods in the country. The program has been welcomed by the community and has shown promising results, which are shaping ongoing discussions about its potential for growth.

“Managing diabetes requires a proactive, personalized approach – not a one-size-fits-all solution,” said Senior Vice President for Medical Management and Chief Health Equity Officer Abdou Bah. “Our program is designed to improve access and intervene early, while also ensuring that those already managing diabetes receive tailored, coordinated care. By offering no-cost education, screenings, and lifestyle classes, we’re building a strong local infrastructure of accessible, reliable support with the goal of improving health outcomes.”

Program highlights from for4 months starting Oct. 1, 2024:

·          4 screening events.

·          460 people screened for diabetes.

·          38 classes on diabetes prevention and management, with a 67% participant completion rate and those who completed the course reported a 100% satisfaction rate.

In March, a new screening event will be held at Neighborhood Care Southern Boulevard, expanding the reach of the program.

EmblemHealth is dedicated to providing tailored, meaningful support for individuals at every stage of their diabetes wellness journey. Efforts include diabetes prevention and management education, exercise classes, nutrition workshops, and emotional well-being courses that are designed to empower members to make incremental and long-term health decisions.
